Linda DeCherrie is the clinical director of Mount Sinai at Home, which brings together multiple Mount Sinai programs that provide in-home care, including Hospitalization at Home, the Mount Sinai Visiting Doctors Program, and Palliative Care at Home. An expert on innovative models of home-based care and medical education concerning chronic care of complex frail patients in the ambulatory setting, DeCherrie is also a professor of geriatrics and palliative medicine at Mount Sinai Health System. She has been with Mount Sinai since completing her fellowship in geriatrics there in 2005.
